We know that medical needs don't stop when office visits are over. Using our help line and our email service we can communicate with patients and troubleshoot issues before your office gets a call. We establish a rapport with patients on behalf of your organization to improve efficiency, revenue and overall patient satisfaction.
Our team and protocols will support you in building a healthier patient base. We ensure age-appropriate screening is complete and documented. We can follow-up on clinical data via labs, remote monitoring, or chronic care management. It's our mission to connect patients with the services they need.
When you choose us, you join a community. We work not just with you but with other members of our community to build a network of people working together for a healthier world.
Remote Patient Monitoring (RPM) is a Medicare benefit. Your provider will offer you a Blood Pressure Monitor, Weight scale or Pulse Oximeter for free. You will take your vitals from the comfort of your home 5 days a week. We will then be able to monitor your vitals from the office and contact you if your vital is ever outside of the normal range. This program will save you a trip to the doctor's office to take vitals.
Participating in RPM helps provide your care team with a more complete picture of your health. This allows them the ability to provide you with more support, education, and feedback on your conditions and treatments. Patients who participate in RPM show continued improvement in the management of their chronic conditions.
Medicare covers this program including the cost of the devices. You are responsible for your usual copay for outpatient medical services. Please refer to your health plan for detailed payment information.
Chronic care management (CCM) is a program available to Medicare beneficiaries with two or more chronic conditions. Your provider will create a plan to address these chronic conditions and provide a team to support you.
Participating in CCM helps provide your care team with a more complete picture of your health. This allows them the ability to provide you with more support, education, and feedback on your conditions and treatments. Patients who participate in CCM show continued improvement in the management of their chronic conditions.
